VENUE RESERVATIONS

  1. Craft the concept paper for your event/activity. Secure required parts of the paper (refer to the concept paper form), and its needed signatures. Afterward, submit it to the Office of the Student Affairs through this link:Concept Paper Form. Submit your response through this link as well.
  2. Once approved, you may proceed with booking your desired venue.Channels may vary depending on your chosen venue:

      1. Fill out the information required on the website. For a heads-up, the website requires you to attach your (1) approved concept paper, (2) proof of approval for the rental fee waiver (if you have emailed the Executive Vice President to request waiving of the fee), and (3) floor plan and program flow if applicable.
      2. After filling out the necessary information, an email will be sent to you confirming your request for booking.
      3. Wait for the approval of the approvers before proceeding.
      4. Once the ending approver has approved, you have successfully and officially booked the venue!

    3. Physical Plant Office for:
      1. Classrooms
        1. Go to the registrar's office and inquire what are the available rooms at the time of your activity. After the confirmation, you will be directed to go to the Physical Plant Office to request a form that you need to complete.
    4. Ma’am Suzanne Doromal, for:
      1. Martin Hall (4th, 6th, 7th Floor)
        1. First, proceed to the Athletics Office located at the back of the 4th Floor, Martin Hall, and inquire about the availability of the venue.
        2. Then, send a letter addressed to Ma’am Suzanne Doromal about reserving any of the Martin Hall floors. The letter can be sent online through her addu email address: smadoromal@addu.edu.ph or submit a physical letter on the 9th floor of the CCFC building.
        3. After the approval of Ma’am Su, proceed to the University Athletics Office to obtain a copy of the Martin Hall reservation form. Attached to the reservation form is the letter approved by Ma’am Su. Before submitting the reservation form back to the University Athletics Office, the necessary information must be inputted, along with the needed signatories.
    5. HISFLEX ROOM RESERVATION FORM for:

        1. Before submitting your reservation request, please obtain the necessary approval from your immediate supervisor and attach a PDF or image of the approval document to the form. No additional paper submissions will be required.

REQUEST FOR OFFICIAL DOCUMENTS

Registrar's Office:
The form caters to both current students and alumni. The students and alumni should fill this out in order to request documents and for drive-through payment and claim of the requested document(s).

Access the link if you intend to request for any of the following documents: Transcript of Records (TOR) for Board Exam (Please upload 2x2 picture) -- Required for licensure examinations.

  • Diploma, (Certified True Copy) -- A certified replica of your diploma.
  • Diploma, Original -- (Issued only once and Subject to verification).
  • Diploma (Second Copy) -- May be denied release for cause. Affidavits of explanation justifying the request must be sent as part of your response to the confirmation email which you will receive subsequently.
  • Reissued LLB to Juris Doctor Diploma -- A replacement or re-issuance of LLB your diploma, per LLB circular.
  • Honorable Dismissal (Subject to verification) -- Only one copy allowed for release (issued only once). This is requested as part of the credentials for students/graduates who desire to transfer to other schools.
  • Certification, Authentication, and Verification from (CAV) CHED (Local)
  • Certification, Authentication, and Verification (CAV) from CHED (Abroad) / DFA
  • Certification: Bona Fide Student -- Certifies the period of attendance of the requesting party as an incumbent student of their specific program.
  • Certification: Complete Academic Requirements (Graduate School)
  • Certification: Eligibility (C1) Authentication for J.D.
  • Certification: Enrollment -- Specify school year and semester in the details.
  • Certification: Grades -- Please indicate in the details the school year, semester, and number of copies.
  • Certification: Graduation -- Certifies the details relative to the grant of the degree by a graduate.
  • Certification: Medium of Instruction (MOI) -- Indicates that the medium of instruction by the University is English, unless otherwise required by law.
  • Certification: Units Earned
  • Certification: QPI / GWPA
  • Course Description -- (Processing time: 2 weeks. Please specify in the details if requesting specific courses (subjects) or all)
  • Letter of no Objection -- Subject to verification.
  • Australian Health Practitioner Regulation Agency Documents -- 2 CTC Diploma, Original TOR abroad with 2 authentications, 2 Letter of Confirmation, Autonomous Certificate (cert. of fee), Certificate of AHPRA, 1 long brown envelope.
